29 CFR  1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about October 31, 2022 the employer had not conducted a periodic inspection of the machine specific procedures since October of 2020. Employees are tasked with service and maintenance of equipment within the facility including but not limited to bottling lines, conveyors, filling lines,  and plastic container manufacturing equipment. Hazardous energies present on equipment include but are not limited to hydraulic, pneumatic, thermal, mechanical, and electrical.  Superior Dairy was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(d)(3), which was contained in OSHA inspection number 1296897, citation number 1, item number 3b and was affirmed as a final order on May 19, 2018 with respect to a workplace located at 4719 Navarre Road, Canton, OH 44706

29 CFR  1910.147(d)(3):All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s):  On or about October 24, 2022  the employer failed to ensure that the energy isolating devices of the TR7 pint and quart milk container filling unit were located and operated to isolate the energy sources. An employee reached into the exit side conveyor line, through a hinged barrier door, and entered the hazardous area of the equipment while it was in operation. The  safety interlock relay switch was not functional at the time. The employee was removing containers which had jammed in the exit portion of the conveyor system. Hazardous energy sources include mechanical, gravitational, and electrical.  Superior Dairy was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(d)(3), which was contained in OSHA inspection number 1534304, citation number 1, item number 1 and was affirmed as a final order on October 7, 2021 with respect to a workplace located at 4719 Navarre Road, Canton, OH 44706  Superior Dairy was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(d)(3), which was contained in OSHA inspection number 1407810, citation number 1, item number 1 and was affirmed as a final order on April 20, 2020 with respect to a workplace located at 4719 Navarre Road, Canton, OH 44706  Superior Dairy was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(d)(3), which was contained in OSHA inspection number 1296891, citation number 1, item number 1 and was affirmed as a final order on May 19, 2018 with respect to a workplace located at 4719 Navarre Road, Canton, OH 44706.  Superior Dairy was previously cited for a violation of this occupational safety and health standard 29 CFR 1910.147(d)(3), which was contained in OSHA inspection number 1296897, citation number 1, item number 1b and was affirmed as a final order on May 19, 2018 with respect to a workplace located at 4719 Navarre Road, Canton, OH 44706
