An employee reached into the exit side conveyor line, through a hinged barrier door, and entered the hazardous area of the equipment while it was in operation. The  safety interlock relay switch was not functional at the time. The employee was removing containers which had jammed in the exit portion of the conveyor system. Hazardous energy sources include mechanical, gravitational, and electrical.
