29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a- Block 700.09, Lot 8, 15 Coliseum Drive, Eastampton NJ: A worker was exposed to struck by hazards when working beneath and near workers performing framing and other related activities without using a protective helmet. Condition observed on or about 06/10/21.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   a- Block 700.09, Lot 8, 15 Coliseum Drive, Eastampton NJ: Workers were exposed to fall hazards, up to and in excess of 10 feet 5 inches to the ground surface below, when using an aluminum extension ladder whose side rails did not extend at least 3 feet above a landing surface being accessed.   Condition observed on or about 06/10/2021.

29 CFR 1926.1053(b)(6): Unsecured ladder(s) were used on surfaces that were not stable and level:  a- Block 700.09, Lot 8, 15 Coliseum Drive, Eastampton NJ:  Workers were exposed to fall hazards, in excess of 10 feet 5 inches to the ground surface below, when using an unsecured aluminum extension ladder that was not set a surface that was stable and level.  Condition observed on or about 06/10/21.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired:  a- Block 700.09, Lot 8, 15 Coliseum Drive, Eastampton NJ: Workers were exposed to fall hazards when using an aluminum extension ladder that was missing both of its factory installed feet and contained several visibly damaged rungs. Condition observed on or about 06/10/2021.

29 CFR  1926.1053(b)(18): Ladder repairs did not restore the ladder to a condition meeting its original design criteria before the ladder was returned to use:  a- Block 700.09, Lot 8, 15 Coliseum Drive, Eastampton NJ: Workers were exposed to fall hazards when using an aluminum extension ladder that was returned to use after its factory installed feet, and the side rails into which they were set, were cut off so that the ladder was set on it bare side rails.  Condition observed on or about 06/10/2021.

Condition observed on or about 06/10/2021.   NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.
