29 CFR 1926.28(a):The employer did not require the wearing of appropriate personal protective equipment in all operations where there is an exposure to hazardous conditions. a) At 17 Pike St, St Charles, MO 63301 - on or about June 17, 2021, four employees of Julio Cesar Guerrero were engaged in residential construction activities without utilizing eye protection. Four employees were not required to wear eye protection while using pneumatic nail guns.

Four employees and the employer were observed on the roof of the building without fall protection, exposing them to a fall to a concrete surface below.
