The program did not enable each employee to recognize the hazards of falling and did not train each employee in the procedures to be followed in order to minimize these hazards: On June 10, 2022, employees of Julio Cesar Guerrero were installing shingles onto the roof of a new residential building located at 2015 Wyndgate Meadows Drive, Lake Saint Louis, Missouri. The employees were not able to recognize fall hazards and the procedures necessary to reduce or eliminate these hazards.

The employer did not require employees to wear eye protection while using Metabo NV45AB2 pneumatic nail guns. Julio Cesar Guerrero was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.28(a), which was contained in OSHA inspection number 1536821, citation number 1, item number 1 and was affirmed as a final order on 11/16/2021, concerning a workplace located on lot 17 Pike Street, Saint Charles, MO 63301.

The employer did not ensure that employees were protected by a guardrail system, safety net system or personal fall arrest system. The employees were exposed to fall hazards while working on the roof of the building which was approximately 11 feet above the lower level with a pitch of 6:12. Julio Cesar Guerrero was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1536821, citation number 1, item number 2 and was affirmed as a final order on 11/16/2021, concerning a workplace located on lot 17 Pike Street, Saint Charles, MO 63301.
