29 CFR 1910.132(d)(1): The employer did not assess the workplace to determine if hazards are present, or are likely to be present, which necessitate the use of personal protective equipment (PPE): On or about June 16, 2021, the employer did not asses the workplace to determine if hazards were present. An employee had his toes smashed by a ramp when it was dropped into place at the gap between the dock and the truck. Employees are exposed to struck-by hazard.
