29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures: Employees were exposed to tripping hazards while working in and walking through piles containing rebar, scrap wood, broken and damaged cinderblocks, sheets of plastic, paper and other debris in the following areas: (a) Outside near the scaffold. Employees must walk through piles of debris to access the scaffold. (b) Inside the houses. Employees must walk through piles of debris to stairs and ladders leading to the roofs of the houses. (c) On Stairways. Employees use stairways to access the roofs of the houses. (d) On the roof. Employees working on the roof of the house installing stripping wood were observed stepping over wood scraps and 2x4s.

Employees were not utilizing a personal fall arrest system (harness) or guardrail system to protect against falls.
