29 CFR  1910.305(b)(2)(i):Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy:  (a)CrossCountry Freight Solutions at 3223 North 1st Avenue Sioux Falls, SD 57104: On or about and at times prior to June 28, 2021, the employer did not ensure employees were protected from electrical hazards in that a wall mounted electrical outlet near the overhead door in the mechanics work area did not have an effective cover to reduce employee exposure to live parts. This condition exposed employees to electrical hazards.

29 CFR  1910.1200(h)(1):Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  (a)CrossCountry Freight Solutions at 3223 North 1st Avenue Sioux Falls, SD 57104: On or about and at times prior to June 28, 2021, the employer did not ensure employees were protected from chemical hazards in that employees that work in the vicinity of hazardous chemicals were not ensured to have been provided effective information and training on hazardous chemicals including Diablo fertilizer, propane, and Diesel 911.  This condition exposed employees to chemical hazards.

29 CFR  1910.178(l)(2)(ii):The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  (a)CrossCountry Freight Solutions at 3223 North 1st Avenue Sioux Falls, SD 57104: On or about and at times prior to June 28, 2021, the employer did not ensure employees were protected from struck-by hazards in that forklift operators did not complete the formal instruction portion of training prior to operation of forklifts onsite. This condition exposed employees to struck-by hazards.
