Located at Lot 30, Earhart Drive, Groveport, Ohio: On or about July 20, 2021, the employer did not ensure that employees operating pneumatic nailers and a circular saw were provided with and utilized eye protection, thereby exposing employees to a struck by hazard.

29 CFR  1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached.  a. Located at Lot 30, Earhart Drive, Groveport, Ohio: On or about July 20, 2021, an employee was utilizing a personal fall arrest harness that was attached to a rope lifeline that was wrapped around a roof truss and tied in a knot to itself. The connection was not an approved anchorage, thereby exposing the employee to a fall hazard.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  a. Located at Lot 30, Earhart Drive, Groveport, Ohio: On or about July 20, 2021, the employer did not maintain an effective safety and health program to ensure that hazards were identified and corrected onsite while employees performed framing work.   Luis Felipe Rosales Arias was previously cited for a repeat violation of this Occupational Safety and Health standard 29 CFR 1926.20(b)(1) which was contained in OSHA inspection number 1505599, citation number 2, item number 1 and was affirmed as a final order on April 12, 2021, with respect to a workplace located at 4267 Salt Spring Lane, Columbus, Ohio 43207.  Luis Felipe Rosales Arias was previously cited for a serious violation of this Occupational Safety and Health standard 29 CFR 1926.20(b)(1) which was contained in OSHA inspection number 1483190, citation number 1, item number 1a and was affirmed as a final order on September 22, 2020, with respect to a workplace located at 3232 Korg Street, Lot 8, Powell, Ohio 43065

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   a. Located at Lot 30, Earhart Drive, Groveport, Ohio: On or about July 20, 2021, employees were sheathing the roof trusses without fall protection, thereby exposed to a fall hazard from approximately twenty feet.   Luis Felipe Rosales Arias was previously cited for a repeat violation of this Occupational Safety and Health standard 29 CFR 1926. 501(b)(13) which was contained in OSHA inspection number 1505599, citation number 2, item number 2 and was affirmed as a final order on April 12, 2021, with respect to a workplace located at 4267 Salt Spring Lane, Columbus, Ohio 43207.  Luis Felipe Rosales Arias was previously cited for a serious violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1483190, citation number 1, item number 1c and was affirmed as a final order on September 22, 2020, with respect to a workplace located at 3232 Korg Street, Lot 8, Powell, Ohio 43065.
