29 CFR 1926.502(d)(15):Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached. a. Located at Lot 30, Earhart Drive, Groveport, Ohio: On or about July 20, 2021, an employee was utilizing a personal fall arrest harness that was attached to a rope lifeline that was wrapped around a roof truss and tied in a knot to itself. The connection was not an approved anchorage, thereby exposing the employee to a fall hazard.
