On or about July 20, 2021, at the above addressed jobsite, employees performing residential carpentry operations, eleven (11) feet three (3) inches above ground, were not protected from falls, thereby exposing the employees to fall hazards.

On or about July 20, 2021, at the above addressed jobsite, employees performing residential carpentry operations were not provided training on how to minimize fall hazards, thereby exposing the employees to hazards associated with falls.
