29 CFR  1926.21(b)(2): The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury:   On or about 20 July 2021, at Estate Donoe construction site, St. Thomas, VI. At a heavy construction site, employees engaged in several construction operations were not trained in the recognition on the hazards related to falls, personal protective equipment, electrical and struck-by hazards and the procedures to control or minimize those hazards.      NOTE:	Certification of abatement must be submitted for this citation item as required by 29 CFR 1903.19(c).

29 CFR 1926.150(c)(1)(vi): A fire extinguisher, rated not less than 10B, was not provided within 50 feet of wherever more than 5 gallons of flammable or combustible liquids or 5 pounds of flammable gas were being used on the jobsite:  On or about 20 July 2021, at storage trailer next to carpentry shop, Estate Donoe construction site, St. Thomas, VI. Employees were exposed to fire hazards while constructing wooden forms utilizing a gas-powered generator with a 25-gal gasoline container next to it with no fire extinguishers available or provided in case of a sudden fire.     NOTE: Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.

29 CFR 1926.404(b)(1)(ii): On a construction site, where an assured equipment grounding program was not utilized, all 120-volt, single-phase, 15 and 20 ampere receptacle outlets which were not a part of the permanent wiring of the building or structure and which are in use by employees did not have approved ground fault circuit interrupters for personal protection:  On or about 20 July 2021, at the carpentry shop, Estate Donoe construction site, St. Thomas, VI. Employees engaged on constructing wooden forms were using a portable power drill and circular saw via extension cord without GFCIs.    NOTE:  Certification of abatement is not required for this citation item since it was submitted prior to citation issuance.

29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.   On or about 20 July 2021, at Estate Donoe construction site, St. Thomas, VI.  Employees engaged on installation of concrete blocks were allowed to work at the edge of the unprotected edge or close to it without proper fall protection system in place at a height more than 15 feet from the ground level.
