The employer reported on Friday, September 2, 2022 a work-related incident that occurred on Friday, August 26, 2022. An employee was removing concrete forms from a current pour, a form fell from the wall, an intact pin hooked the worker's safety vest and pulled the worker from the scaffold to the ground. NOTE: Certification of abatement is not required for this citation item as abatement was documented by the compliance safety and health officer during the inspection.
