A Mack Truck with a concrete conveyor belt attached (No Data Plate Available) was not checked at the beginning of each shift to ensure that the following parts, equipment, and accessories were in safe operating condition and free of apparent damage, which could cause failure while in use: service brakes, including trailer brake connections; parking system (hand brake); emergency stopping system (brakes); tires; horn; steering mechanism; coupling devices; seat belts; operating controls; and safety devices, exposing employees to a collision hazard.  (b) On or about 5/23/2023, at the Paul E. Joseph Stadium in Frederiksted, St. Croix, VI 00820. Joseph Stadium in Frederiksted, St. Croix. - an employee was sitting on the ledge without fall protection at approximately 10 feet (3 Meters) in height, exposing employees to a fall hazard.  The GEC, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 1544419, citation number 1, item number 5, and was affirmed as a final order on Jan 26, 2022, with respect to a workplace located at Estate Donoe construction site, St. Thomas, VI.
