29 CFR 1926.652(a)(1):  Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652(b) or (c):  (a)	GLH Construction, LLC, at the NW corner of S. Timberline Rd. and Rosen Dr., Fort Collins, CO 80528: On July 29, 2021, the employer did not ensure that each employee conducting work in an excavation was protected from cave-in, in that work performed to install concrete and blue poly pipes for a yet to be constructed concrete diversion vault was done in an area of the trench that was approximately 10 feet deep with a near-vertical wall on the north side. The excavation was not sloped or benched and lacked any kind of protective system.  This condition exposed employees to an engulfment and/or struck-by hazard from trench excavation wall cave-in.   GLH Construction, LLC was previously cited for a violation of the same or substantially similar condition of this Occupational Safety and Health Standard 29 CFR 1926.652(a)(1) which was contained in OSHA Inspection Number 1236442, Citation Number 1 Item 1 issued on September 8, 2017 and became Final Order on September 27, 2017 with respect to a workplace located at Centre Avenue and Research Boulevard, Fort Collins, CO 80526.
