29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: (a) At the temporary jobsite located near the intersection at 37th Ave Ct and W 4th St, Greeley, CO 80634: Employees were exposed to struck-by hazards in that the employer did not ensure that a daily inspection of the trench was conducted prior to employees working inside. The trench had nearly vertical walls, water at the bottom of the trench due to a leaking water line, and was approximately 6.5 feet in depth.

(b) At the temporary jobsite located near the intersection at 37th Ave Ct and W 4th St, Greeley, CO 80634: Employees were exposed to struck-by hazards in that two employees were inside the trench as well as inside the trench box while it was being lowered by the boom of an excavator.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): (a) At the temporary jobsite located near the intersection at 37th Ave Ct and W 4th St, Greeley, CO 80634: Employees were exposed to struck-by hazards in that the employer did not ensure that a protective system was utilized while employees worked inside a trench that had nearly vertical walls, water at the bottom of the trench due to a leaking from a water line, and was approximately 6.5 feet in depth. GLH Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.652(a)(1), which was contained in OSHA inspection number 1545041, citation number 1, item number 1 and was affirmed as a final order on February 9, 2022, with respect to a workplace located at the NW corner of S. Timberline Rd & Rosen Dr., Fort Collins, CO 80528.
