29 CFR  1926.501(b)(13):"Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   a. Located at Building 5, 40-60 Preswicke Mill in Blacklick, Ohio: On or about August 2, 2021, the employer did not ensure that subcontractor employees were protected from fall hazards while performing roofing work from the second story roof of the muli-unit condominium building, thereby exposing subcontractor employees to fall hazards from approximately twenty foot.   b. Located at Building 6, 16-36 Preswicke Mill in Blacklick, Ohio: On or about August 2, 2021, the employer did not ensure that subcontractor employees were protected from fall hazards while performing roofing work from the second story roof of the muli-unit condominium building, thereby exposing subcontractor employees to fall hazards from approximately twenty foot.   c. Located at Building 7, 8208-8220 Wyncrest in Blacklick, Ohio: On or about August 2, 2021, the employer did not ensure that subcontractor employees were protected from fall hazards while performing roofing work from the second story roof of the muli-unit condominium building, thereby exposing subcontractor employees to fall hazards from approximately twenty foot.   d. Located at Building 7, 21-23 Green Mill in Blacklick, Ohio: On or about August 2, 2021, the employer did not ensure that subcontractor employees were protected from fall hazards while performing roofing work from the second story roof of the muli-unit condominium building, thereby exposing subcontractor employees to fall hazards from approximately twenty foot.   e. Located at Building 8, 46-66 Green Mill in Blacklick, Ohio: On or about August 2, 2021, the employer did not ensure that subcontractor employees were protected from fall hazards while performing roofing work from the second story roof of the muli-unit condominium building, thereby exposing subcontractor employees to fall hazards from approximately twenty foot.   Tase, Inc was previously cited for a serious violation of this Occupational Safety and Health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1490093, citation number 1, item number 2 and was affirmed as a final order on 10/27/2020, with respect to a workplace located at 1634 Heather Lane, Middleburg Heights, Ohio 44130.
