29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: Main Warehouse, near Bay 4: A DAYTON commercial pedestal fan (model NSC-243, 2.3A) power supply cord was missing the ground prong. Employees were exposed to electrical hazards.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that the strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws. Meat Department: Flexible cords and cables were not provided strain relief as required. The cables was missing approximately 2 inches of insulation exposing the energized wires to possible damage. Employees were exposed to electrical hazards.
