29 CFR 1910.22(a)(3): Walking-working surfaces were not maintained free of hazards such as sharp or protruding objects, loose boards and general slip or trip hazard. At the entrance of the freezer, the metal threshold was bent and uneven, with phillips-head screws sticking out above the rest of the metal. Employees are exposed to uneven walking conditions at the entrance of the freezer and could slip and fall.
