29 CFR 1910.147(c)(6)(i) The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed. (a) On or about October 10, 2021, at the worksite located at W220 N507 N Springdale Road, Waukesha, WI, the employer did not ensure a machine specific lockout procedure was developed and implemented for servicing and maintenance activities of machines including, but not limited to a Machinenbau stock mounter #2 machine, thereby exposing employees to struck-by and caught-in-between hazards.
