29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about February 2, 2025, the employer had not ensured that the, Tape Machine, (Model: USA5024TB-OPF3, Serial No. TM641 20D 032), was equipped with adequate guarding to ensure employees could not access the "in-running" nip point, exposing employees to an amputation hazard.

29 CFR 1910.303(b)(7)(iv): There shall be no damaged parts that may adversely affect safe operation or mechanical strength of the equipment, such as parts that are broken, bent, cut, or deteriorated by corrosion, chemical action, or overheating. (a) On or about February 5, 2025, the employer did not effectively remove or replace the damage outer sheathing on the Silver Stitcher s#200001HP, thereby exposing employees to a 120-volt electrical hazards. (b) On or about February 5, 2025, the employer did not effectively remove or replace the damage outer sheathing on the Thompson Platen Press, thereby exposing employees to a 120-volt electrical hazards.
