29 CFR 1910.212(a)(2) Guards shall be affixed to the machine where possible and secured elsewhere if for any reason attachment to the machine is not possible. The guard shall be such that it does not offer an accident hazard in itself. a) On or about August 15, 2021 the employer did not ensure that guarding on the Press 2 cold extrusion would not in itself create a hazard. Employees were exposed to the retracted saw blade beneath the table, in that, the present sensing matt that controlled the safeguards on the saw failed to retract, which did not prevent access to the point of operation and the saw blade.
