29 CFR 1910.179(n)(3)(i): The load was not well secured and properly balanced in the sling or lifting device before it was lifted more than a few inches: On or about February 11, 2025, at a worksite located at 12051 Mahoning Avenue, North Jackson, OH 44451, the employer did not ensure all loads being lifted and moved by the 2 ton overhead crane were secured. An employee was moving bolster 11-20 into a staging area when the eye bolt became dislodged allowing the bolster to fall.

29 CFR 1910.184(c)(9): Employee(s) were not kept clear of load(s) suspended, or about to be lifted by sling(s): On or about February 11, 2025, at a worksite located at 12051 Mahoning Avenue, North Jackson, OH 44451, the employer did not ensure employees were kept clear of all suspended loads. An employee was moving bolster 11-20 into a staging area holding onto the suspended load when the eye bolt became dislodged allowing the bolster to fall.
