29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about August 21, 2021 the employer failed to ensure that procedures for the isolation of hazardous energies of the P.M. Wire Drawing machine were utilized prior to service and maintenance taking place. Employees were not removed from and able to be within the hazardous area of the machine while positioning wire on coil take-up blocks. Hazardous energies present on the equipment mechanical, electrical, pneumatic, and high pressure water.

29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy had not been put into place. On or about August 26, 2021 the employer failed to ensure that all specific procedural steps were contained in the machine specific procedures of the P.M. Wire Drawing Machine. The employer failed to include steps for ensuring that rotating coil blocks had come to a complete stop before proceeding with the remainder of the LO/TO procedural steps.
