29 CFR 1910.502(c)(1):The employer did not develop and implement a COVID-19 plan for each workplace with all the required site specific information. a) Phoenix Indian Medical Center located at 4212 N 16th St, Phoenix, AZ 85016 On or about September 28, 2021, the employer failed to develop and implement a COVID-19 plan to mitigate the spread of SARS-CoV-2, the virus that causes Coronavirus Disease 2019 (COVID-19). Employees working in the ICU, Emergency Room, and Medical/Surgical Unit providing care to suspected and/or confirmed COVID19 patients, were not provided with the required site-specific information, such as adequate workplace-specific policies and procedures.

29 CFR 1910.502(c)(4)i: The employer did not conduct a workplace-specific hazard assessment to identify potential workplace hazards related to COVID-19. a) Phoenix Indian Medical Center located at 4212 N 16th St, Phoenix, AZ 85016 On or about September 28, 2021, the employer had not conducted a workplace-specific hazard assessment to identify potential workplace hazards related to COVID-19 and aerosol generating procedures in the Emergency Department. Hazards included, but are not limited to: direct patient care such as patient screening, taking vitals, administering non-invasive ventilation, cardiopulmonary resuscitation, and other patient-facing tasks. b) Phoenix Indian Medical Center located at 4212 N 16th St, Phoenix, AZ 85016 On or about September 28, 2021, the employer had not conducted a workplace-specific hazard assessment to identify potential workplace hazards related to COVID-19, including aerosol generating procedures in the Intensive Care Unit. Hazards included, but are not limited to: direct patient care such as, extubation, administering non-invasive ventilation, taking vitals, feeding, bathing, and other patient-facing tasks. c) Phoenix Indian Medical Center located at 4212 N 16th St, Phoenix, AZ 85016 On or about September 28, 2021, the employer had not conducted a workplace-specific hazard assessment to identify potential workplace hazards related to COVID-19 and aerosol generating procedures in the Medical-Surgical Unit. Hazards included, but are not limited to: direct patient care such as taking vitals, administering non-invasive ventilation, feeding, bathing, and other patient-facing tasks.
