Employees were exposed to a fall hazard of 8 feet approximately while accessing a loft area using an extension ladder that extended 12-inches approximately above landing surface.

Employees were exposed to a fall hazard of 8 feet approximately to floor level while working in a loft area that lacked a midrail on all exposed sides.
