(a) Hull 127 - On or about September 1, 2021 and at times prior thereto, employees were exposed to being struck-by falling tools, equipment and/or materials when transiting under and around the barge hull section without adequate toeboards.

29 CFR 1915.53(c): Precautions were not taken to prevent ignition of highly flammable hardened preservative coatings. (a) Hull 126 & 127 - On or about September 1, 2021 and at times prior thereto, employees were exposed to insipient stage fires and/or vapor inhalation in that preservative coatings were not removed from the backside of the steel hull prior to torching hull cuts.
