29 CFR 1926.501(b)(4)(i):  Each employee on walking/working surfaces was not protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels by personal fall arrest systems, covers or guardrail systems erected around such holes:           a) On or about August 30, 2021, employees were exposed to the hazard of falling approximately 16 feet through unprotected skylights located throughout the roof deck work area.      No abatement certification or documentation required for this violation.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system;     a) On or about August 30, 2021, employees were exposed to the hazard of falling approximately 16 feet while performing roofing work along the edge of the roof without the use of fall protection or a safety monitor.   b) On or about August 30, 2021, employees were exposed to the hazard of falling approximately 16 feet while performing roofing work and throwing debris from the edge of the roof without the use of fall protection or a safety monitor.        No abatement certification or documentation required for this violation.

29 CFR 1926.502(f)(1)(iii): Points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines:      a) On or about August 30, 2021, employees were exposed to the hazard of falling approximately 16 feet where warning lines were not used to create access paths connecting the work area to the point of access along the roof edge.   b) On or about August 30, 2021, employees were exposed to the hazard of falling approximately 16 feet where warning lines were not used to create access paths connecting the work area to the debris removal area along the roof edge.         No abatement certification or documentation required for this violation.
