29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a. On or about 05/13/2021 at #3 Oxford Court, Suffern NY 10901-  Employees were working below bundles of roofing shingles that were conveyed to the roof by a material hoist. The employees working below the bundles were not wearing protective helmets and were exposed to  overhead impact hazards.

29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a. On 05/13/2021- # 3 Oxford Court, Suffern NY 10901 CSHO observed  employee using electric circular saw to cut roof rafters (Photo # 5762) using an electric circular saw exposed to eye loss/eye injuries as eye protection was not utilized. Condition observed on or about 05/13/2021    b. On 05/13/2021- #3 Oxford Court, Suffern NY 10901, CSHO observed an employee installing a gutter system and ice shield using a Metabo pneumatic nail gun was exposed to eye loss/eye injuries due to struck by hazards as eye protection was not utilized. (Photo # 5776, #5787) Condition observed on or about 05/13/2021

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels is not protected by guardrail systems, safety net systems, or personal fall arrest systems.   The employer does not require the use of any form of fall protection when employees are performing residential construction activities 6 feet or more above lower levels. The violation was most recently observed on or about May 13, 2021, at a residential construction site located at or near #3 Oxford Court, Suffern, New York 10901, where four employees were not protected from fall hazards of approximately eighteen feet to the ground. To abate this violation, the employer must ensure that its employees performing residential construction activities 6 feet or more above lower levels are protected from falls, and that no employee is allowed to perform residential construction activities at heights greater than 6 feet above lower levels without adequate fall protection.  Pursuant to 29 CFR 1903.19, within 30 days the employer must submit documents describing the steps it is taking to ensure Compliance, including a description of how these steps protect its employees from fall hazards
