29 CFR  1910.147(d):  The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about September 14, 2021, and October 7, 2021, employees were exposed to amputation hazards while performing tooling changes/die changes on press brakes and power presses, and the employer failed to implement energy control application steps as the machines  were not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements}.  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

On or about September 14, 2021, and October 7, 2021, employees exposed to potential amputation hazards from the daily operation of mechanical power presses within the facility using presence sensing devices for employee protection, and the employer did not equip the power press with a test rod of a diameter specified by the presence sensing device manufacturer to represent the minimum object sensitivity of the sensing field, with instructions for use of the test rod noted on a label affixed to the presence sensing device.

29 CFR  1910.217(h)(10)(ii):  The following checks were not made at the beginning of each shift and whenever a die change is made.  On or about September 14, 2021, and October 7, 2021, employees exposed to potential amputation hazards from the daily operation of mechanical power presses within the facility using presence sensing devices for employee protection, and the employer did not conduct the following checks at the beginning of each shift and whenever a die change is made:  a.	A check must be performed using the test rod according to the presence sensing device manufacturer's instructions to determine that the device is operational, b.	The safety distance must be checked to for compliance, c.	A check must be made to determine that all supplemental safeguarding is in place, d.	A check must be made to assure that the barriers and/or supplemental presence sensing devices are operating properly, and e.	A system or visual check must be made to verify correct counterbalance adjustment for die weight according to the press manufacturer's instructions, when a press is equipped with a slide counterbalance system.
