A stairway, ladder, ramp or other safe means of egress shall be located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees.  On or about October 4, 2021, at a job-site located at the intersection of Windfall Rd at SR 18, Medina, Ohio 44256; the employer did not ensure employees had means of access and egress while working inside a trench that had near vertical side walls and a depth of approximately 10 feet. An employee was working in the 10 foot deep trench connecting a 12 inch waterline and did not have means of access and or egress. The company had a 16 foot portable ladder laying in the parking lot to the East of the trench.
