29 CFR 1926.501(a)(2):The employer did not determine if the walking/working surfaces on which its employees were to work on, had the strength and structure integrity to support employees safely: a. On or about October 5, 2021, at the above addressed jobsite, an employee was performing roofing operations on a gymnasium roof, when the roof section the employee was working on collapsed, exposing the employee to the hazards associated with falls.

29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: a. On or about October 5, 2021, at the above addressed jobsite, the personal fall arrest system used by an employee conducting roofing operations was rigged to allow a free fall greater than six (6) feet and did not prevent the employee from contacting a lower level.
