Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems: On 10/4/2021, employees of HR Vasquez Construction LLC were performing roofing work at 1006 Regency Manor Drive, Ballwin, Missouri. Employees were working on a residential roof approximately 20 feet above a lower level. Employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection.

An extension ladder was placed on the West side of the house and used to access the upper landing surface of the roof. The side rails of the ladder did not extended 3 feet above the upper surface.
