29 CFR  1903.19(d)(1):The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  (a)	The employer failed to provide documentation demonstrating the abatement method and completion for employees conducting roofing activities on a roof not protected from falling to the lower level issued on 12-16-2021 as Citation 1 Item 1.   (b)The employer failed to provide documentation demonstrating the abatement method and completion for training to recognize and minimize the fall hazard issued on 12-16-2021 as Citation 1 Item 2.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  (a)  On or about October 15, 2021, employees conducting roofing activities on a roof with a 6:12 pitch were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 18 feet on the front of the house and 25 feet on the back of the house.  Barry's Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1501177, citation number 1, item number 1 and was affirmed as a final order on December 21st, 2020, with respect to a workplace located at 2171 Langford Ln., Wheaton, IL 60187.  Barry's Construction, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1516073, citation number 2, item number 1 and was affirmed as a final order on March 31, 2021, with respect to a workplace located at 25901 76th Place, Salem, WI 53168.
