29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about October 13, 2021, an employee was exposed to a struck by hazards while performing siding activities on a commercial building located at 8199 York, North Royalton, Ohio 44133.  An employee was working above another employee with tools and materials that could have fallen on a lower employee and the employer did not ensure that the lower employee was wearing a protective helmet.

29 CFR  1926.102(a)(1): 29 CFR 1926.102(a)(1):  Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents:  On or about October 13, 2021, an employee was exposed to a struck by hazard while performing siding activities on a commercial building located at 8199 York, North Royalton, Ohio 44133.  An employee was using an air powered nail gun and the employer did not ensure that he was wearing protective safety glasses.

An employee was working from a JLG 450AJ articulating lift and the employer did not ensure that he was using fall protection.

On or about October 13, 2021, an employee was exposed to a fall of approximately 27 feet while performing siding activities on a commercial building located at 8199 York, North Royalton, Ohio 44133.  An employee was working on a 3rd floor balcony and the employer did not ensure that he was using fall protection.
