Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about October 8, 2021, at a worksite located at 220 Drott Street in Schofield, Wisconsin, fall protection was not utilized while employees were pouring concrete on an elevated working surface exposing employees to fall hazards of approximately 10.75 feet. b. On or about October 15, 2021, at a worksite located at 220 Drott Street in Schofield, Wisconsin, fall protection was not utilized while employees were marking the elevated concrete surface exposing employees to fall hazards of approximately 10.75 feet.

On or about October 15, 2021, at a worksite located at 220 Drott Street in Schofield, Wisconsin, employees were on a walking/working surface and were not protected from falling through holes approximately 10.75 feet above a lower surface level without utilizing conventional fall protection equipment.

On or about October 15, 2021, at a worksite located at 220 Drott Street in Schofield, Wisconsin, employees were not protected by hole covers on the concrete deck. Employees were exposed to the hazard of tripping into approximately 25 holes on their walking surface that measured approximately three inches in diameter. b. On or about October 15, 2021, at a worksite located at 220 Drott Street in Schofield, Wisconsin, employees were not protected by hole covers on the concrete deck. Employees were exposed to the hazard of stepping into approximately 26 holes measuring approximately seven inches in diameter.

29 CFR 1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. On or about October 15, 2021, at a worksite located at 220 Drott Street in Schofield, Wisconsin, the employer did not provide required fall protection safety training for employees exposed to fall hazards greater than six feet while marking lines prior to framing walls on the new construction.
