29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)   On or about  February 9, 2022, in the Converting Room, at  the Flat Bag machine #50, the employer did not insure that barrier, light curtain or other guarding was in place to protect employees from an ingoing nip point created by the seal bar and machine surface. This condition exposed workers to crushing and amputation hazard.

29 CFR 1910.219(a)(2): Vertical and inclined belts (paragraphs (e) (3) and (4) of this section) if not more than two and one-half (2 1/2) inches wide and running at a speed of less than one thousand (1,000) feet per minute, and if free from metal lacings or fastenings were not guarded with a nip-point belt and pulley guard.   (e) On or about February 9, 2022, In the Converting Room, at  Converting Machine #6, the drive belts for the top  roller assembly were not properly guarded to protect machine operators from the hazard of ingoing nip point.  This condition exposed workers to an amputation Hazard.

29 CFR  1910.219(f)(1): Gear(s) were not guarded by a complete enclosure or by one of the methods specified in 29 CFR 1910.219(f)(1)(ii) and (f)(1)(iii):  a) On or about February 9, 2022, at the 40 inch 6 color  printing machine, the drive gear enclosure guarding was not fully closed and fastened in place.  This exposed employees to a pinch point and amputation hazard from the drive gear system.

29 CFR 1910.1200(f)(6)(ii): Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  (a) On or about October 20, 2021, In the Converting Department, at converting machine #2, the employer did not ensure the red  five (5) gallon capacity metal safety can containing solvent was labeled, tagged, or marked with information such as but not limited to the chemical identity and associated hazard warnings.  The Converting Technology, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.1200(f)(6), which was contained in OSHA inspection number 1181334, citation number 2, item number 1 and was affirmed as a final order on  4/18/2017, with respect to a workplace located at 7737 North 81st Street Milwaukee WI. 53223.
