29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a)  On or about September 28, 2022, in the converting room at converting machine #8, the employer did not insure guarding was properly implemented, allowing the employee(s) to access the powered rollers and zingers in an energized state. This exposed the employee(s) to ingoing nip point, crushing hazard and amputation hazard.  In the alternative...  29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a)  On or about September 28, 2022, in the converting room at converting machine #8, the employer did not insure hazardous energy control lockout procedures were utilized during machine maintenance functions that required employee to re-web the energized machine. This exposed employees to ingoing nip points, crushing hazard and amputation hazard created by the energized powered rollers and zingers.  The Converting Technology, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 1579998, citation number 1, item number 1 and was affirmed as a final order on August 5, 2022, with respect to a workplace located at 7737 North 81st Street in Milwaukee, WI.
