29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) On or about February 17, 2022, in the converting room at converting machine #4, the employer did not insure hazardous energy control lockout procedures were utilized during machine maintenance functions that required employee to re-web the energized machine. This exposed employees to ingoing nip points, crushing hazard and amputation hazard created by the energized powered rollers and zingers. (b) On or about February 17, 2022, in the converting room at converting machine #7, the employer did not insure hazardous energy control lockout procedures were utilized during machine maintenance functions to include a process that required employee to re-web the energized machine. This exposed employees to ingoing nip points, crushing hazard and amputation hazard created by the energized powered rollers and zingers. In the alternative... 1910.212(a)(1): Machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts and crushing hazards. (a) On or about February 17 , 2022, in the converting room at converting machine #4, the employer did not insure guarding was properly implemented, allowing the employee(s) to access the powered rollers and zingers in an energized state. This exposed the employee(s) to ingoing nip point, crushing hazard and amputation hazard. (b) On or about February 17 , 2022, in the converting room at converting machine #7, the employer did not insure guarding was properly implemented, allowing the employee(s) to access the powered rollers and zingers in an energized state. This exposed the employee(s) to ingoing nip point, crushing hazard and amputation hazard.
