On or about 10/12/2021, Fernando Flores DBA Fernando Flores was removing and replacing roofing materials on a roof of a residential building located at 776 Ryan Lane in Foristell, MO. An employee was installing roofing materials with a pneumatic nail gun without wearing eye protection.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest systems. On or about 10/12/2021, Fernando Flores DBA Fernando Flores was removing and replacing roofing materials on a roof of a residential building located at 776 Ryan Lane in Foristell, MO. They were not utilizing guardrail systems, a safety net system, or personal fall arrest systems. The owner and thee employees were observed working on the roof, approximately eight to fourteen feet above the ground, without utilizing fall protection.
