29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On July 10, 2023, employees of Fernando Flores dba: Fernando Flores were removing old shingles and installing new roofing shingles onto a residential building located at 404 Tulleries Court in Foristell, Missouri. Employees were using pneumatic nail guns without eye protection. The employees were exposed to struck-by hazards. Fernando Flores dba, Fernando Flores was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1559159, citation number 1, item number 1 and was affirmed as a final order on January 10, 2022, with respect to a workplace located at 776 Ryan Lane in Foristell, Missouri.

Employees engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest systems: On July 10, 2023, employees of Fernando Flores dba: Fernando Flores were removing old shingles and installing new roofing shingles onto a residential building located at 404 Tulleries Court in Foristell, Missouri. The employer did not ensure that fall protection was worn while work was being performed on the roof approximately 11-30 feet above the lower level. The employees were exposed to fall hazards. Fernando Flores dba: Fernando Flores was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1559159, citation number 1, item number 2 and was affirmed as a final order on January 10, 2022, with respect to a workplace located at 776 Ryan Lane, Foristell, Missouri.
