29 CFR  1910.178(l):  This is applicable to construction employment as referenced by 1926.602(d). The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):   The employer is failing to protect employee(s) engaged in powered industrial truck operations from crushing and struck-by hazards. This was most recently documented on October 14th, 2021 in Elkhorn, Nebraska. The forklift operator had not completed forklift training to include a classroom portion, and written test that meets the requirements of OSHA standard 1910.178(l) powered industrial trucks, operator training.

The employer is failing to protect employees from struck-by hazards. This was most recently documented on or October 14th 2021, Elkhorn, Nebraska. An employee was engaged in residential framing activities using a skill saw. This employee was exposed to struck-by hazards because he was not utilizing eye protection.

The employer is failing to protect employees from fall hazards. This was most recently documented on or about October 14th, 2021 in Omaha, Nebraska 68022. Multiple  employees were observed working on a residential structure conducting framing operations. The exposed employees were standing on top of the top plates/trusses of the residential structure at heights over 16 feet- No fall protection was being utilized.
