Spencer, MA. 01562 metal polishing garage:  On or about September 24,  2021,  and at times prior, an employee was allowed to perform work tasks such as changing electrical fuses in a 480/277 fuse disconnect box while the circuits were live and had not been provided with training on electrical safe work practices required when completing such work.

29 CFR  1910.157(d)(6):The employer shall provide portable fire extinguishers for Class D hazards which are required in those combustible metal working areas where combustible metal powders, flakes, shavings, or similarly sized products are generated at least once every two weeks:  a) 57 Wire Village Rd. Spencer, MA. 01562 metal polishing garage:  On or about  September  24, 2021, and at times prior the employer had not provided portable fire extinguishers for Class D hazards when employees were working in areas where combustible metal powders were allowed to accumulate and create a potential fire and explosion hazard. Employer had provided an ABC rated fire extinguisher.
