29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.  Employees were exposed to struck-by hazards. Employees engaged in framing activities with the use of pneumatic nail guns were exposed to struck-by hazards. Employees were not provided with eye protection.  The KTM Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1541644, citation number 1, item number 3 and was affirmed as a final order on 10-05-2021, with respect to a workplace located at 13373 W 144th Terrace Olathe, Kansas 66051.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   Employees were exposed to fall hazards. Employees  engaged in framing activities were exposed to a fall distance of approximately 12 ft. from the roof to the ground below. Employees were not using fall protection.  The KTM Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1541644, citation number 1, item number 5 and was affirmed as a final order on 10-05-2021, with respect to a workplace located at 13373 W 144th Terrace Olathe, Kansas 66051.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the jobsites located at 16677 165th Terrace, Olathe, Kansas, employees engaged in residential framing activities were exposed to falls from approximately 12 feet from the roof to the ground below, in that the aluminum ladder used to access the roof did not extend  at least 3 feet above the eave of the roof.  The KTM Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1541644, citation number 1, item number 6 and was affirmed as a final order on 10-05-2021, with respect to a workplace located at 13373 W 144th Terrace Olathe, Kansas 66051.
