29 CFR  1926.416(e)(1): Worn or frayed electric cords or cables were used:   At the jobsite located at 16673 S Tomashaw Street, Olathe Kansas, employees were exposed to shock and arc hazards in that the extension cord used was taped in seven different sections while plugged into a circular saw without proper repair.

29 CFR  1926.1052(c)(1)(ii):Stairways having four or more risers or rising more than 30 inches (76 cm), whichever is less, were not equipped with a stair rail system along each unprotected side or edge:  At the jobsite located at 16673 S Tomashaw Street, Olathe Kansas, employees engaged in framing activities on the second floor of the building were exposed to fall hazards in that the stairway was not equipped with a handrail.

29 CFR  1926.1052(c)(12):Unprotected sides and edges of stairway landings were not provided with guardrail systems that meet the criteria contained in Subpart M of 29 CFR 1926:(a) Note:  Guarding of landings for stairways and ladders are being maintained under Subpart M.  At the jobsite located at 16673 S Tomashaw Street, Olathe Kansas, employees engaged in framing activities on the second floor of the building were exposed to fall hazards in that a guardrail system was not in place around the stairwell opening to prevent employees from fall hazards.

The KTM Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1541644, citation number 1, item number 5 and was affirmed as a final order on 10-05-2021, with respect to a workplace located at 13373 W 144th Terrace Olathe, Kansas 66051   The KTM Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1561931, citation number 2, item number 2 and was affirmed as a final order on 11-22-2021, with respect to a workplace located at 12687 S. Wrangler St. Olathe, KS 66061.  The KTM Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1560100, citation number 2, item number 2 and was affirmed as a final order on 12-09-2021, with respect to a workplace located at 15413 W. 167th St. Olathe, KS 66062.  The KTM Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1560381, citation number 1, item number 2 and was affirmed as a final order on 1-19-2022, with respect to a workplace located at 16677 165th Terrace Olathe, Kansas 66061.  The KTM Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1563598, citation number 2, item number 3 and was affirmed as a final order on 2-10-2022, with respect to a workplace located at 112680 S. 29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed.  At the jobsites located at 16673 S Tomashaw Street, Olathe Kansas, employees engaged in residential framing activities were exposed to falls from approximately 23 feet from the roof to the ground below, in that the aluminum ladder used to access the roof did not extend  at least 3 feet above the eave of the roof, only extending to the 20 foot mark.  The KTM Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1560381 , citation number 1, item number 3, and was affirmed as a final order on 1-19-2022 with respect to a workplace located at 16677 165th Terrace Olathe, Kansas 66061.
