Employees working with and around Ferric Chloride in the etching department were exposed to hazards such as, but not limited to, skin irritation and skin sensitization.

29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Five Handicap, Inc. doing business as Mansfield Graphics located at 127 North Walnut Street in Mansfield, Ohio 44902: Instance A On or about October 20, 2021, the employer did not ensure employees were protected from rotating bits when using the Bridgeport Series 1 Vertical mill located in the machining shop. Employees utilizing the mill to counter sink and counter bore holes were exposed to caught in hazards in that the chuck was not guarded. Instance B On or about October 20, 2021 the employer did not ensure employees were protected from rotating bits when using the JET Floor Drill Presses located in the machining shop. Employees utilizing the drill presses to drill holes were exposed to caught in hazards in that the chuck was not guarded. Instance C On or about October 27, 2021 the employer did not ensure employees were protected from rotating bits when using the JET Jet Step Pulley Drill Press located in the machining shop. Employees utilizing the drill presses to drill holes were exposed to caught in hazards in that the chuck was not guarded.

29 CFR 1910.147(c)(1):The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a) Five Handicap, Inc. doing business as Mansfield Graphics located at 127 North Walnut Street in Mansfield, Ohio 44902: Instance A On or about October 26, 2021, the employer did not develop and implement an energy control program consisting of training, procedures, and audits to protect employees from hazardous mechanical and pneumatic energy sources. Employees that performed tasks such as, but not limited to, clearing and replacing water jet tips on the OMAX 2652 Jet Machining Center were exposed to laceration and amputation hazards in that the energy sources were not isolated. Instance B On or about October 22, 2021, the employer did not develop and implement an energy control program consisting of training, procedures and audits to protect employees from hazardous mechanical and pneumatic energy sources. Employees that performed tasks such as, but not limited to, cleaning internal nozzles of the Chemcut Corporation etching machine are exposed to laceration hazards in that energy sources were not isolated. Instance C On or about October 22, 2021 the employer did not develop and implement an energy control program consisting of training, procedures, and audits to protect employees from hazardous mechanical energy. Employees that change out bits on the Jet Step Pulley Drill Press were exposed to caught in and amputation hazards in that the electrical energy source was not isolated.

29 CFR 1910.1200(g)(8): The employer did not maintain in the workplace copies of the required safety data sheets for each hazardous chemical: a) Five Handicap, Inc. doing business as Mansfield Graphics located at 127 North Walnut Street in Mansfield, Ohio 44902: On or about October 27, 2021, the employer did not maintain, in the work place, copies of the safety data sheets for each chemical hazard. The safety data sheet on hand by the employer for ferric chloride, used in the etching department, was dated April 5, 2004. Employees were exposed to hazards associated with ferric chloride such as, but not limited to, skin irritation and skin sensitization, in that employees did not have access to the most up to date information provided on a safety data sheet under the global harmonizing system.
