29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) Five Handicap, Inc. dba Mansfield Graphics located at 127 N. Walnut Street, Mansfield, Ohio, 44902: On or about June 17, 2025, the employer failed to ensure machine specific lockout tagout procedures were developed, documented and utilized for employees tasked with performing work such as, but not limited to, set up and die changes on the Phoenix hydraulic presses (30 ton, 2 ton, and 12 ton). Without the benefit of specific procedures to control hazardous energy, employees were exposed to caught-in hazards.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: a) Five Handicap, Inc. dba Mansfield Graphics located at 127 N. Walnut Street Mansfield, Ohio, 44902: On or about June 17, 2025, the employer failed to ensure that employees who setup and maintain the Phoenix hydraulic presses were adequately trained in the skills required for the safe application, usage and removal of energy controls. Employees were not trained to isolate and render energy controls safe prior to performing setup tasks.
