29 CFR 1910.119(j)(4)(ii): Inspections and testing procedures performed on process equipment to maintain its mechanical integrity did not follow recognized and generally accepted good engineering practices (RAGAGEP): This violation was most recently observed on or prior to October 27, 2021 in the Honeywell Aclon Unit located in Geismar, Louisiana when the employer did not ensure that inspections and tests performed on process equipment follow recognized and generally accepted good engineering practices (RAGAGEP) such as Ultrasonic Testing (UT)/material thickness testing, including but not limited to: Inlet and outlet piping associated with Pressure Safety Valves (51134, 51234, 51334 and 51434) for Reactors R-511, R-512, R-513 and R-514.

29 CFR 1910.119(j)(4)(iii): The frequency of inspections and tests of process equipment was not consistent with applicable manufacturers' recommendations and good engineering practices (RAGAGEP), and more frequently if determined to be necessary by prior operating experience: Honeywell G-125 Unit: On or prior to October 27, 2021, the employer did not perform inspection and tests on the Emergency Relief Scrubber Tank (U-1950), part of the emergency shutdown system, on a frequency consistent with their chosen RAGAGEP, API 653 Tank Inspection, Repair, Alteration and Reconstruction (Fourth Edition, April 2009) Section 6.3.1-Section 6.4.2 or more frequently after metal thinning was found in 2011 and repairs were made to address corrosion which involved welding a metal band around the outside perimeter of the tank.
