29 CFR  1926.451(b)(1)(i):Each platform unit (e.g., scaffold plank, fabricated plank, fabricated deck, or fabricated platform) was not installed so that the space between adjacent units and the space between the platform and the uprights were no more than 1 inch (2.5 cm) wide, except where the employer can demonstrate that a wider space is necessary.  a) Jobsite - The working level of the tubular welded frame scaffold was decked with spaces larger than one (1) inch thus exposing employees to a fall hazard of approximately twenty-four (24) feet.  Observed on or about 07 OCT 2021.

29 CFR  1926.451(c)(2):Supported scaffold poles, legs, posts, frames,  or uprights did not bear on base plates and mud sills or other adequate firm foundation:  a) Jobsite - The employer did not provide a scaffold that rested on baseplates, mudsills, or any other material that provided a firm foundation when the employees were building a stair tower from the scaffold.  Employees were exposed to fall hazards of approximately twenty-four (24) feet in the event of a scaffold collapse.  Observed on or about 07 OCT 2021.

29 CFR  1926.451(e)(1):When scaffold platforms were more than 2 feet above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers, stairway-type ladders, ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:  a) 1823 Callowhill St, Philadelphia, PA - The employer did not ensure that employees had proper access to the platform level of the erected fabricated frame scaffold exposing employees to fall hazards, approximately 24 feet above the ground  during ascent and decent from the working platform. Observed on or about on or about October 7, 2021.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:  a) 1823 Callowhill St, Philadelphia, PA - The Employer failed to provide fall protection for employees working next to residential foundations and stairway core exposed to falls of approximately 12 feet.  Observed on or about on or about October 7, 2021  The New Team Contractor Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 1926.501(b)(1) , which was contained in OSHA inspection number, 1538950, citation number 01, item number 002 and was affirmed as a final order on September 22, 2021, with respect to a workplace located at 63 E. Main St, Marlton, NJ 08053.
