East scaffolding: An Employee was pulling electrical wire while standing on mobile scaffolding that was not fully decked, and was exposed to fall hazards. b. West scaffolding: Employees were painting the wall from a scaffolding that was not fully decked, and were exposed to fall hazards.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person: East scaffolding: The mobile scaffolding railing was used as an anchor point for a personal fall arrest system. Employee was exposed fall hazards.

29 CFR 1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. East scaffolding: The scaffolding braces were used to access the top of the mobile scaffolding to pull electrical wire. Employee was exposed to fall hazards.
