At 9399 Ribault Avenue, in Jacksonville, Florida 32208; On or about November 2, 2021, the employer exposed employees to head  injuries, in that protective helmets were not utilized by employees working at ground level and beneath other employees working from the roof area.

At 9399 Ribault Avenue, Jacksonville, Florida 32208; On or about November 2, 2021, the employer exposed employees to eye injuries, in that eye protection was not utilized by employees who were operating pneumatic nail guns and portable hand saws while performing nailing and wood cutting operations.

At 9399 Ribault Avenue, Jacksonville, Florida 32208; On or about November 2, 2021, the employer exposed employees to 10-foot fall hazards, in that employees used an extension ladder that did not extend 3-foot above the landing for access and egress from the roof.

29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a. At 9399 Ribault Avenue, Jacksonville, Florida 32208; On or about November 2,2021, the employer is exposing employees to fall hazards ranging from 9 to 17-foot, in that fall protection is not being utilized by employees who are installing sheathing sheets on a 5:12 pitch roof.   Vivas Frameng, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1249589, citation number  1, item number 1 and was affirmed as a final order on August 24, 2017, with respect to a workplace located at 9853 Sailor Drive, Lot 73 - Jacksonville, Florida 32221.
