29 CFR  1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a. At a worksite located at 3314 Warrington Street, in Jacksonville, Florida 32254:  On or about February 2, 2022, the employer exposed employees to head injury hazards, in that protective helmets (hard hats) were not utilized by employees working at ground level and beneath other employees working from the roof and truss areas, while performing framing operations.

At a worksite located at 3314 Warrington Street, in Jacksonville, Florida 32254: On or about February 2, 2022, the employer exposed employees to eye injury hazards, in that eye protection was not utilized by employees who were operating pneumatic nail guns to nail down plywood, while performing framing operations.

At a worksite located at 3314 Warrington Street, in Jacksonville, Florida 32254: On or about February 2, 2022, the employer exposed employees to fall hazards in that, the employer allowed employees to use the top step of a Werner brand, 6-foot potable A-Frame type ladder, to gain access and egress from the trusses, while performing framing operations.

29 CFR  1926.501(b)(13):  Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section:   a. On a 5:12 pitched roof, at a worksite located at 3314 Warrington Street, in Jacksonville, Florida 32254: The employer fails to assure effective protection against falls as required by the standard, as demonstrated on February 2, 2022, when two employees installing sheets of sheathing on the roof of a structure were exposed to the hazard of falls ranging from 9 to 17-foot to the ground below, in that fall protection was not utilized, while performing framing operations.  Vivas Frameng, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1249589, citation number  1, item number 1 and was affirmed as a final order on August 24, 2017, with respect to a workplace located at 9853 Sailor Drive, Lot 73 - Jacksonville, Florida 32221.
